PROBLEM TO BE SOLVED: To provide a record notebook with a pouch in which a mount is made to correspond every packaging of the medicine being taken, and a space pouch of the medicine can be kept after taking the medicine.SOLUTION: A medicine-taking record notebook 1 with a pouch has many mounts where a left end is closed and each is provided in response to one-time medicine-taking. In a right half of the mount 2, there is provided a transparent plastic pouch 4 where a zipper seals liquid-tightly an opening of the pouch in a freely opening/closing manner. In a left half of the mount, there is provided a memorandum column 5 of the medicine-taking record concerning a name of the medicine, usage, the usage date and time, and making checks by classifying the pain when in use into the pain of multiple steps. The medicine is put in the plastic pouch 4 by one pack, and thereby the memorandum column of one piece of the medicine-taking record is made to correspond per one pack, and the space pouch after the medicine-taking is returned to the original plastic pouch 4 and can be stored.SELECTED DRAWING: Figure 1COPYRIGHT: (C)2018,JPO&INPIT【課題】服用する薬の個装毎に台紙を対応させ、服用後も薬の空き袋を保管できる袋付きの記録ノートを提供する。【解決手段】 袋付き薬服用記録ノート1は、左端が閉じられ夫々が一回の服用に対応して設けられた多数の台紙を有し、台紙2の右半分にはジッパーが袋の開口を開閉自在に液密状にシールする透明のプラスチック製袋4を備え、左半分に薬の名前、使用量、使用日時、使用したときの痛みを複数段階の痛みに分けていずれかをチェックさせる服用記録のメモ欄5を設けている。1包ずつプラスチック製袋4に入れておくことにより、一包につき、一枚の服用記録のメモ欄を対応させ、服用後の空き袋をもとのプラスチック製袋4に戻すことにより保管できるようにした。【選択図】 図1
