PROBLEM TO BE SOLVED: To provide a medical observation apparatus capable of reducing a burden of an operator by controlling an image of an affected section and an operative section in an observation screen to an appropriate brightness even if various treatment instruments are taken in/out with respect to an operative field.

SOLUTION: This medical observation apparatus is provided with an observation means 2 having at least one image-pickup device for capturing a subject image through an objective lens, a support section 3 movably and fixedly supporting the observation means 2 at a desired position, a light modulation means photometically measuring the brightness of the subject image and controlling the modulation, an observation state detection means 11 detecting fluctuation and a stopped state of an observation view field of the observation means 2, and a modulation control changeover means changing the modulation control of the subject image to a different control by the light modulation means according to the detection result of the observation state by the observation state detection means 11.
