PROBLEM TO BE SOLVED: To allow patients by themselves to easily manage information without requiring large-scaled systems, and to allow the information to be easily shared with medical treatment persons concerned.SOLUTION: An information terminal device 100 includes: a short-range wireless communication 101 that receives a request of a communication permission to be transmitted from an information terminal device 200 an operation display unit 102 that receives an input of whether to permit the communication in response to the request of the communication permission and a page creation unit 103 that, when receiving to the effect that the communication is permitted, acquires medical consultation history information from a prescription information storage unit 110, and creates a medical consultation history page from the acquired medical consultation history information, in which the short-range wireless communication 101 transmits the medical consultation history information to the information terminal device 200. The operation display unit 102 displays the medical consultation history page created by the page creation unit 103.
