Included are a synchrotron (130) which accelerates a charged particle beam and a respiration sensor generating a respiration signal such that the respiration signal monitors a breathing cycle of the patient. A rotatable platform holds the patient rotates through about three hundred sixty degrees during an irradiation period of the patient. The synchrotron (130) uses the respiration signal to deliver the charged particle beam to the tumor at a set point in the breathing cycle. The delivery of the charged particle beam at the set point of the breathing cycle occurs in greater than five rotation positions of the rotatable platform.
