A multidirectional self-retaining suture (1100) is disclosed. The suture (1100) comprises a grasp engagement element (1106) and at least three suture segments (1102). Each suture segment (1102) has a plurality of retainers disposed on portions of the suture adjacent the ends (1104). The retainers (1103) permit movement of the suture through tissue in a first direction and prevent movement in a direction opposite the first direction. The multi-directional suture (1100) is suitable for closing complex wounds. A bidirectional suture with two ends is also disclosed.
