In one aspect of the invention, an apparatus has a shaft having a distal end and a tip at the distal end of the shaft. The tip has an opening formed in the surface of the tip. An instrument inserted through the shaft extends distally outside the opening. The device further comprises a lift for engaging the instrument. The lift includes a proximal end secured to the surface of the tip, a proximal portion extending distally from the proximal end, and a distal portion extending distally from the proximal portion.
