Das Transportmedium ist ein bei Raumtemperatur festes Lösungsmittel für den Wirkstoff； je nach Polarität des Wirkstoffs ist das Lösungsmittel entweder ein hydrophiles Lösungsmittel - für hydrophile Wirkstoffe - oder ein hydrophobes Lösungsmittel - für hydrophobe Wirkstoffe. Wird das Pflaster auf die Haut geklebt, so schmilzt das feste Lösungsmittel aufgrund der - gegenüber Raumtemperatur - erhöhten Körpertemperatur.
