The present invention relates to a method for displaying a 3D image and a radiography guide system and method using a 3D image. The radiography guide system using a 3D image according to the present invention includes an overlay image providing unit. The overlay image providing unit provides an external image showing a part of a body of a virtual patient as the 3D image, provides an internal image showing a skeletal structure of the part of the body part of the patient as the 3D image, provides a radiological image to photograph the part of the body of the virtual patient with radiation, and provides two or more of the external image, the internal image, and the radiological image on a screen to be overlaid with a matching state.
