The present invention relates to an apparatus for facilitating treatment via a vascular wall defining a vascular lumen containing an occlusion therein, the apparatus comprising an intravascular device having a lumen and a distal portion, the distal portion including at least one aperture and a reentry device having a body with a proximal portion and a distal portion, and a penetrator extending distally beyond a distal end of the body, wherein a diameter of the body is greater than a diameter of the penetrator, wherein the reentry device has a natural state and a compressed state, wherein when the reentry device is in the compressed state, the distal portion is substantially aligned with the proximal portion, whereby the reentry device is configured to be advanced into the lumen of the intravascular device, and wherein when the reentry device is in the natural state, the distal portion forms an angle with respect to the proximal portion, whereby the distal portion of the reentry device is configured to be advanced through the at least one aperture.
