Insulin analogs contain a truncated B-chain polypeptide that lacks the B1-B3 residue, and optionally further substitutions in the C-terminal B23-B30 region of the B chain. The insulin analog lacking the B1-B3 residue may optionally contain a substitution at B28 and / or B29 that confers a fast-acting action and a non-standard substitution at B24. The analog may be an analog of mammalian insulin such as human insulin. Nucleic acids encoding such insulin analogs are also provided. The method of treating a patient comprises administering to the patient a physiologically effective amount of an insulin analog or a physiologically acceptable salt thereof.
