PROBLEM TO BE SOLVED: To provide a handle adapter excellent in fitting characteristics and removal characteristics with respect to a stem impactor, which hardly causes rattling in a rotational direction, and a surgical instrument for an IBG procedure equipped with the handle adapter.SOLUTION: A handle adapter 1 includes an inner rod 2, an outer rod 3, and a hook part 4. When the hook part 4 is caused to slide to one end part 33a side of a through-hole 33 that the outer rod 3 includes, the tip part 2a of the inner rod 2 protrudes from the tip part 3a of the outer rod 3. When the hook part 4 is caused to slide to the other end part 33b side of the through-hole 33, the tip part 2a of the inner rod 2 is accommodated inside the outer rod 3.
