The invention relates to a method for custom-making a preformed implant, arranged to fix with a screw a first part of a bone to a second part of this bone which must be separated by an osteotomy, and to a preformed guide for drilling holes for the screws and for guiding the osteotomy pattern, said method comprising the steps of, prior to the osteotomy, using a computer to perform a pre-operative model of the osteotomy； complete bone, constituting a pre-operative form of said complete bone, and modifying said pre-operative model, so as to create a modified model.
