PROBLEM TO BE SOLVED: To provide an adaptor capable of holding a distal end of a barrel with higher holding power, and to provide a barrel with the adaptor.SOLUTION: An adaptor 1 for attaching a medical instrument to a distal end part of a barrel for a syringe comprises multiple holding sections 3 which hold the distal end of the barrel in cooperation with one another and are arranged at intervals in a circumferential direction along an outer surface of the distal end part of the barrel. Each of the holding sections has: a contact part that comes into contact the distal end part of the barrel and a proximal end part that is successively provided to the outside of the contact part in a radial direction of the distal end part of the barrel. The contact part is formed so that a thickness in a centerline direction orthogonal to the circumferential direction and the radial direction larger becomes larger than that of the proximal end part in the center line direction, and has guide surfaces for guiding the distal end of the barrel toward the distal end part side of the contact part.
