The present invention relates to a device capable of applying electrotherapy massage to head, neck and shoulders, including: an inner core structure which is made of a foam material, having a horizontal accommodating opening formed in a center thereof and configured to allow a patient's neck to be accommodated therein stably and comfortably so as to make an inner edge of a rear portion that of that is near a rear side positioned against the patient's neck and also to allow two side arms thereof positioned against the patient's shoulders, the inner core structure also including at least two vertical cushion openings and a plurality of horizontal cushion openings, wherein each of the vertical cushion openings is vertically opened in the rear portion to provide the rear portion with capability of deforming front or back, and each of the horizontal cushion openings is horizontally opened in the side arms and the rear portion to provide the inner core structure with enough ventilation capability； at least four protruding portions made of electrical conductive fibers, having hardness greater than that of the inner core structure, and having an edge sewn to or embedded in an inner wall of each horizontal cushion opening, with one side thereof protruding in a direction toward the horizontal accommodating opening to allow each electrical conductive fiber to form a current conduction relationship with the skin of the user's neck or shoulders； a plurality of electrical wires, each electrical wire having an end that is connected to each electrical conduction fiber via the inner core structure and another end connected to an electrotherapy signal generating apparatus to receive electrotherapy pulse signals transmitted from the electrotherapy signal generating apparatus to transmit the electrotherapy pulse signals to portions of the patient's neck or shoulders corresponding thereto via each electrical conduction fiber； and an outer covering which is a sleeve made of breathable fa
