СПОСОБ ПРОФИЛАКТИКИ ВНУТРИПЛЕВРАЛЬНОГО КРОВОТЕЧЕНИЯ В ИНТРАОПЕРАЦИОННОМ И РАННЕМ ПОСЛЕОПЕРАЦИОННОМ ПЕРИОДАХ ПОСЛЕ ОПЕРАЦИИ ПО ПОВОДУ ТУБЕРКУЛЕЗА ЛЕГКИХ И ПЛЕВРЫ
The method for preventing intraoperative and early postoperative intrapleural bleeding following surgery for the tuberculosis of lungs and pleura comprises the thorough hemostasis by electrocoagulation and the tamponage with the gauze swabs soaked in the hot isotonic solution. Furthermore, Aprotinine at a dose of 500,000 KIU is administered intravenously intraoperatively. Then the troubled areas of the lung and the thoracic wall are laid round with the gauze swabs soaked in Aprotinine solution. On the first day postoperatively, Aprotinine at a dose of 200,000 KIU every 6 hours is administered intravenously. On the second day, the dose of Aprotinine is 200,000 KIU thrice a day. On the third day – 200,000 KIU twice a day. In the case of the massive intraoperative diffuse hemorrhages, the freshly frozen plasma of the same group is transfused in a volume of 5-10 mL/kg for 30 minutes. In 8 hours postoperatively, the freshly frozen plasma of the same group is transfused in a volume of 5 mL/kg when the hemoglobin content in exudate exceeds half of the blood hemoglobin content. At the end of the surgery, sanitization fibrobronchoscopy is provided. The drainages are connected to aspiration immediately after bringing the ribs together. The lungs are gradually inflated.Способ профилактики внутриплеврального кровотечения в интраоперационном и раннем послеоперационном периодах после операции по поводу туберкулеза легких и плевры включает тщательный гемостаз электрокоагуляцией и тампонадой марлевыми салфетками, смоченными горячим физиологическим раствором. Дополнительно интраоперационно вводят 500000 КИЕ апротинина и обкладывают марлевыми салфетками, смоченными раствором апротинина, проблемные участки легкого и грудной стенки. В послеоперационном периоде в первые сутки назначают внутривенно апротинина по 200000 КИЕ каждые 6 ч, по 200000 КИЕ три раза в день - на вторые сутки, по 200000 КИЕ два раза в день - на третьи сутки.
