The present invention provides a pneumatic cylinder with vacuum suction and pressure regulation, which comprises a pneumatic cylinder, which is configured therein with a fixed isolation plate, and a piston member slidably disposed inside the fixed isolation plate. The center of the piston member is configured with an airtight piston with an air volume adjusting screw, and the end is configured with a second airtight piston. As such, the interior of the pneumatic cylinder is formed with a first air chamber, a second air chamber and a third air chamber that all the air chambers are connected with a sleeve. When the piston member is moving, the air may flow into and out of the pneumatic cylinder, such that the volumes of the air chambers can be adjusted corresponding to the space. The variation of air chamber volumes may vacuum the interior of the sleeve, so that the stump and the prosthetic may be tightly sucked together and the weight feeling of the prosthetic during walking may be greatly reduced, and the pressure of each air chamber may be balanced and naturally stabilized in response to walking operation of the user.
