A new method for implanting a cardiac treatment system involves implanting the system lead under the sternum in front of the heart without attaching it to the myocardium or pericardium. Examples include the placement of the anchor under the sternum, the position above the ventricle, the region surrounded by the second or third rib, and one of the adjacent sternums at the same height as the aortic arch. A tension element or tether is attached to the anchor and a lead is introduced on the tension element or tether and secured in a desired position relative to the anchor.
