PROBLEM TO BE SOLVED: To provide a medical image transmission system capable of immediately transmitting a requested image when requested from a terminal device, and performing smooth inspections and diagnosing, and to provide a gate server in the system.SOLUTION: A medical image transmission system of one example includes: a modality for capturing a medical image a gate server for attaching incidental information of processing based on a DICOM protocol, tentatively storing the medical image obtained by the modality as DICOM image data, and then transmitting it a processing server for receiving the DICOM image data transmitted from the gate server and processing the DICOM image data on the basis of the incidental information of the processing and a terminal device for specifying a processed image and requesting transmission to the gate server. When the image requested to be transmitted by the terminal device is not turned to a transmittable state from the processing server, the gate server processes the image requested to be transmitted in the gate server, and transmits it to the terminal device.
