A device for delivering a drug for rheumatoid arthritis through the dermal barrier, comprising: a microneedle and a plurality of nanostructures made on the surface of a microneedle, nanostructures form a predetermined pattern; a remedy for rheumatoid arthritis in fluid communication with a microneedle. 2. The device according to claim 1, wherein the pattern further comprises microstructures, wherein the nanostructures have a sectional size smaller than the microstructures. The device according to claim 2, additionally containing second nanostructures with a cross-sectional size smaller than the cross-sectional size of the microstructures and larger than the first cross-sectional nanostructures. The medical device according to claim 1 or 2, wherein at least a portion of the nanostructures has a sectional size of less than about 500 nanometers and more than about 5 nanometers. The medical device according to claim 1 or 2, wherein at least a portion of the nanostructures has a gap between the centers of the nanostructures from about 50 nanometers to about 1 micrometer. The medical device according to claim 1 or 2, wherein at least a portion of the nanostructures has a height of from about 10 nanometers to about 20 micrometers. The medical device according to claim 1 or 2, in which at least in some area of the nanostructure have a size ratio of from about 0.15 to about 30.8. The device according to claim 1 or 2, wherein the medicament for rheumatoid arthritis is a mitigating disease antirheumatic drug.
