One end of the image positioning plate 33 is brought into contact with or close to the distal end D1 of each tooth of the image target dentition D1 and an image carrier 34 is formed in such a shape as to cover the one end of the image positioning plate 33 and the distal end Da of the tooth of the image target dentition D1. Then and the carrier 4 is carved and created by a carving device based ON the shape of the image carrier 34.
