An implant for therapeutic separation of joint bone has two end plates each having an opening through the end plate and at least one inclined surface on the opposite side of the bone engaging side. The frame is slidably connected to the end plate, allowing the end plate to move relative to each other at an angle relative to the longitudinal axis of the implant in a sliding connection with the frame. An actuator screw is rotatably connected to the frame. The carriage forms an open area aligned with the opening in the end plate. The opening in the end plate forms an unobstructed passage through the carriage from bone to bone of the joint.
