Disclosed are systems and methods for closing a tissue defect. The system may be placed percutaneously between two skin access sites, a subcutaneous guide, and advanced through the first access site into the body by a first needle, and then passed to a second needle. A self-locking strap. The strap may be withdrawn with a second needle and out of the body through the second access site. The strap may be withdrawn from the first needle, leaving the strap in place, through both access sites, across the defect. As the subcutaneous guide is withdrawn from the body, the captured strap can be pulled and strapped around the defect such that both ends of the strap protrude from the first access site. A device for closing a tissue defect is also disclosed.
