PROBLEM TO BE SOLVED: To provide an X-ray image diagnostic apparatus an image transfer method capable of drawing a region of interest in detail while drawing the whole image even when the amount of data of an X-ray image receiver exceeds the limit of the transfer speed.SOLUTION: In an X-ray image diagnostic apparatus 1 with a photographing device 10 and an operation device 20 (display device 25) connected with each other via a communication cable 29 (transmission line), the photographing device 10 comprises an image compressor 16 which compresses and transfers X-ray images. The image compressor 16 determines the size of a ROI (region of interest) according to the frame rate of a fluoroscopic image, the standard number of thin-outs, and the transfer limit value. In addition, thinning-out processing is not performed in the inner region of a ROI, and in the outer region of the ROI, the thinning-out processing is performed based on the standard number of thin-outs to generate a compressed image with the amount of data decreased, and transfers the compressed image to the operation device.
