Devices and systems provide a surgical access device (300) comprising an internal retractor device (370) integrated with or coupled to a body wall (40) or wound retractor (350). The wound retractor (350) retracts an opening in a body wall (40) into a body cavity (42), while the internal retractor (370) permits a user to control the positions of internal structures (30) within the body cavity (42), thereby permitting a user to define a surgical field (70). Embodiments of the internal retractor (370) are adjustable.
