A system comprising: a cannula having a substantially cylindrical shape defining an interior space, the cylindrical shape having an axis of a cylinder; an intermediate material located in the interior space; an optical fiber located in the intermediate material, the optical fiber having an optical axis of the fiber and a distal end configured to emit light, the emitted light having a distribution pattern of illumination with an illumination axis that is not parallel to the axis of the cylinder. 2. The system of claim 1, wherein at least a portion of the optical axis of the fiber closest to the distal end is not parallel to the axis of the cylinder. The system of claim 1, wherein the distal end of the optical fiber is a shaped distal end configured to change the optical axis of the fiber. The system of claim 1, wherein the distal end of the cannula is configured to make incisions. The system of claim 1, wherein the cannula is configured to retract so that the distal end of the optical fiber protrudes more than the distal end of the cannula.
